REQUEST FORM FOR COUNSELLING SERVICES
Below you can find request forms for counselling services at the Educational and Diagnostic Centre.
QUESTIONNAIRES FOR PARENTS AND TEACHERS
All the school or legal guardian of a child have to do is download the relevant questionnaire and fill it in (it can also be filled in on your PC by just clicking on the relevant options).
Please print out the questionnaire and have both the legal guardian of the child and the school sign it.
The school or parent can then send the questionnaire by mail to the address of PPP Step ahead of the planned evaluation or the parent can bring it with them to the evaluation.

In exceptional cases (acute problem solving, etc.) it is possible to agree with the investigating psychologist to fill in the questionnaire only at the time of the examination.
CONSENT TO PERSONAL DATA PROCESSING
Please only fill out the consent to personal data processing if you have been asked to do so by the evaluating psychologist (before class observations, preparation of documentation etc.). Otherwise the counselling centre staff member will fill in the consent after you arrive for the evaluation.
